The ability to ambulate or walk is an extremely important element in functional mobility. A cane is a type of gait assist device that allows an individual with lower extremity strength deficits, minor balance and stability issues, health conditions, injury, or pain to walk with greater safety, thus maintaining this important element of functional independence.
A Few Basics of Cane Use
Fit of a Cane
- For proper height, the handle of the cane should be at the level of the wrist when it hangs down freely and with the elbow in slight flexion
- The handle of the cane should be comfortable to grip and to hold while weight bearing
Walking with a Cane
- The cane is held by the hand “opposite” the leg that is injured, weak, or limited
- Place the cane on the ground before placing weight on the cane
- When stepping forward with your “involved/injured” leg, swing the cane forward contacting the ground so it is even with the tip of the foot
- Place pressure on the cane to reduce the pressure on the involved leg
- Step forward with the good leg moving past the cane. Repeat with the gait cycle
- When turning, pivot with good leg, not the involved leg
Walking Up or Down Stairs with a Cane
- Note: if there is a handrail, use it
- Going Up: Step up with the good leg first. Place weight on the good leg and extend upward, bringing the (in other hand) cane and involved leg up to the level of the good leg. Take steps one at a time.
- Going Down: Set the cane down. Bring involved leg down using the cane for support and balance. Take steps one at a time.
- To help remember the above cane use guidelines, the statement, “The good go up to heaven, and the bad go down to hell” is often referenced.
