REUSABLE STANDARD AND LARGE ALLEN TRIFOLD SCORING CARD
The Allen Cognitive Level Screen (ACLS) and Large Allen Cognitive Level Screen (LACLS) requires a screening tool to assess the level at which a patient can function independently. This Scoring Table Trifold is covered in plastic for multiple uses that is better for the environment and meant for long term use. Simply wipe the table clean when finished with a patient’s assessment. Folded up, it measures 4.75" x 8.5", making it easy to take from assessment to assessment. The chart is in English only.
